A Case of Ophthalmoplegia and Blepharoptosis Mimicking Incomplete Oculomotor Nerve Palsy Following Injection of Filler into the Nasal Bridge

Abstract
Injectable facial fillers are frequently used in cosmetic treatments, but various adverse effects are reported from small nodule formation or bluish discoloration under the skin to acute vision loss or cerebral ischemic stroke. We report a 26-year-old female patient who had binocular diplopia, left side ocular pain, headache, and dizziness after facial filler injection. Bilateral visual acuity and pupillary reflexes were normal, but incomplete ptosis, medial gaze palsy and inferior gaze palsy of the left eye were observed. The combination of above eye symptoms seemed like an incomplete oculomotor nerve palsy. However, magnetic resonance images(MRI) showed contrast enhancement of left medial rectus muscle, left inferior rectus muscle and left orbital soft tissue. According to the MRI, selected ophthalmoplegia without visual loss after filler injection mimicked incomplete oculomotor nerve palsy and was caused by focal extraocular muscle myositis.
